Dear lifeylife...

I've been mengeluh too much...
Waking up at 4, taking a bus at 5 and later an angkot to the hosp every morning has tire me up a little bit.

Arriving the hosp around 6, running while shouting to own self, "I'm late! I'm late". Makin kuat perasaan nak mengeluh bile tgok pintu pagar depan hosp tak buka pun lg which forces me to lari ke pintu belakang hosp which makes me another 3 mins late.

Arrive at the labour room, sigh heavily the moment I saw few families waiting out side the labour room. It must be a tough night for my fellow colleagues who stayed back for the oncall last night. Silently I prayed that tonight will be a calm night for me(oncall night baybeh)..

I was slapped back into reality when my colleague told me that a patient will be brought up to the labour room later so she has to stay back at the labour room a little bit later before going up to do patients follow up. Hoyay! More patients for me(sarcastic)...

Oncall tonight, esok visite dgn dr E. Ade kemungkinan kne maki habis2an coz I heard he was in such in an ill tampered lately. 32 working hours stright is actually nothing. I am so grateful.

Xperlu lah nak mengeluh bnyk sngt awa... Ade org yg kerja dia lagi berat, risiko lagi besar dan dpt gaji xberbaloi pun nak bandingkan dengan kita. Kutip sampah, lombong arang batu, cuci baju rumah jiran, sapu pasir kt jalanan dan macam2 jenis pekerjaan lagi yg xterfikir oleh otak cetek ni. Kalau kita asek mengeluh je bermaksud kita xbersyukur. Whining won't makes everything bare able. Smiling and being grateful makes everything seems light and easy.

Dear lifeylife...

A young girl, came to the obs-gyn clinic, complaining of having vaginal bleeding and was really concern of her 14-week-old-first-pregnancy. I sent her to the gyn chair, checked where were the bleeding from and confirmed it came from the little opening of the cervix. "It might be a miss-carriage", I told her, and I could see her blank eyes went watery and filled with sadness. "But let's just confirm it with a transvaginal USG, ok?", somehow, I felt a rush of guilty feeling filled my empty heart when I saw a little hope in her smile when she nodded her head.

Moments later, her turn for the USG came. She eagerly stole a glance at the monitor in every little opportunity she had, hoping to see her little baby move, but immediately she was sent to a very great despair when we confirmed the miss-carriage.

I patted her forearm, "you are still very young, don't be sad, there will be another one soon, don't worry", and then explained to her that we need to do a curretage to make sure the bleeding stops.

Truthfully, I know, every time I said "it's going to be ok", it will not change much, coz she had lost her baby she hoped for. It is sad u know, the one who really wants a baby, ended up having a miss-carriage, but the one who didn't, gave birth to a very healthy baby and ended up leaving the baby near a dustbin. Life is so unfair...

Pernahkah anda mmberus pakaian yg sedang anda pakai?
Ye, saya baru melakukah hal tersebut, tahniah buat saya.
Itu lah akibatnya kalo ade air ketuban merecik secara berleluasa pada satu2nya seluar oncall yg dibawa ke hospital.
Survival rate: boleyla.

Td saya jd assistant time c-sect. Dan melakukan pemotongan fallopian tube. Saya ketagih.

salam...
life at the OBGYN department is surely hectic as i thought it would be
there are 6 people in our group and everyday, 2 of us have to stay back at the hospital for oncall
that means we have to do oncall every two nights.

oncall night*labor room, wards, operation theater(OT), ER and whatnots*
postcall night*exhausted-sleep early*
precall night*stress-sleep early* ;p
oncall night*ctrl+c then ctrl+v*

last week was my OT's week
so i spent most of my time in the OT
mostly for C-sects and curettes and other laparatomy operations for gynee patients
nothing much basically because it was my first OT rotation so i just help with the patient's pre-op preparation, paper works and helping with the baby

the most i could do was being an assistant during the curette which was no fun at all
cant wait for my next OT rotation where i'll be able to be the 1st assistant during a c-sect

this week i was assigned to be at the labor room
meaning more and more vagina touche, episiotomy+sew it back and of course being as supportive as we could be in time of leading tired-helpless-desperate mommies undergoing their pain during the harsh stages of labor.

but at the end, listening to the first cry of a new-born baby on his/her effort to inhale as much oxygen as he/she could to make sure the lungs inflate well for the blood to enter the lungs for the first time and start the complete blood circulation, see the blueish face turned pink and see he/she actively moves his/her limbs for the first time, surely make every thing worth it all!*manly-tears* ;p

td selepas solat isya sementara tunggu nak solat tarawikh, imam tu ade kasi kuliah 7minit(dont ask me why '7 minit' coz i oso dont know=))

he said, manusia makhluk berakal, oleh itu manusia ade lupa.

if tgh solat and tiba2 kita lupa kita berada di rakaat ke berapa, maka ambil lah rakaat yang terkecil

which means, if kita tak sure either rakaat 3 or 4, so teruskan lah solat seperti kita dalam rakaat 3, dan pada tahiyat terakhir, lakukan lah sujud sahwi....

hal yang diterangkan tadi dilakukan pada solat yang telah ditetapkan rakaatnya contohnya solat fardhu. kalau solat yang tidak ditetapkan rakaat nya, contohnya solat tarawikh, tak perlu lah nak tambah rakaat lagi, so boleh solat seperti biasa sahaja. if nak tambah rakaat pun boleh sebagai penyempurna solat.

Smlm I stayed up until 3am settle kan status patient ujian. Sbb today I kne exam patient. Pastu tibe2 kene coverkan colleague oncall arini coz die kene exam patient die tomorrow. And unfortunately, oncall arini xsetenang oncall 2 hari lepas. Td pg ade baby apnea(stopped breathing) and we cpr-ed for more then 1 hour. Baby tu kembar. The bigger baby yg apnea. The smaller one still bertahan. Tonite at around 7, the bigger one tu apnea lg, then akhirnye meninggal. At the same time the smaller one started having difficulty to breath. Its gonna be a very loonngg loonngg nite tonite!

Ps: ni kaki the smaller baby. Kecik kan?
Ps lg: nak tdo kejap tp xle sbb kne observe baby yg critical every 30 mins. Mata da rabak!

had a superbly unproductive post-call sunday

like i always said, post-call syndrome --> hyper-tido-isme

but today was beyond acceptation

end up waking up feeling soooo stressful

ah watever

salah sendiri rite*otak ligat carik alasan utk diri sdiri*

buuuut i had such a hard-core on-call schedule for the past 3 weeks

kerja for 14 days straight with night shift 36 hours every 2 days for almost 3 weeks and 24 hours sunday shift 2 weeks in a row is a little bit too much for me. for me yg pemalas ni la ;p

heh watever la

will finish my pediatric cycle in 3 weeks bebeh!

nomore screaming babies oyeh!

nomore stubborn parents oyeh!

nomore small legs kicking me whenever i want to put the small stethoscope onto their small chest!

nomore morbilli+bronkopneumonia, nomore diarrhea+dehydration, nomore anemia+undernourished, nomore febrile seizures, nomore DSS, nomore encephalitis morbilli, nomore cyanotic babies.... nooomooooree!!(for now at least)

but i'll be missing all those sweet smiles, innocent faces.

putting my both hands into a patient's tummy during a laparotomy is sooooo cooolll

sooo soooo coool!

holding the slimy colon from the inside of tummy is cooler

and when my consultant asked me to hold the warm liver, i felt like i might pingsan with excitement

closing up the laparotomy on my own while my consultant did the colostomy is superfragilicious

however

feeling that the cancer has literally closed up the rectum and climbing up to the colon is bad

holding the warm liver but realize that the cancer has mestastasized from the rectum and now is eating up the liver is far worst

but closing up without doing nothing but the colostomy because the cancer has literally taking over the body and there's nothing we could do to stop it is devastating

sometimes, some things are just meant to be....

pagi yg berat mmbawa mata yg membengkak ke ward sambil cuba menyorokkan mata dibalik spek... walau sebenarnya tak berjaya...

pagi bertambah berat bila patient kesayangan diberitakan meninggal jam 12 malam semalam

berat menyesak di dada sbb hari terakhir jumpe patient tersebut jumaat lepas, beliau masih tersenyum ceria despite having full thickness rectal prolapse and total uterine prolaps

lebih berat rasa masa oncall hari sabtu, i was too preoccupied dengan emergency operation acute appendicitis dr Eko SpB...

bertambah-tambah berat bile actually after op dengan dr Eko selesai, i sempat di suruh follow up patient dr Solya SpB yg berada di ward yang sama dengan my patient itu... but i was so busy with dr Solya's patient smpai i lupe bertegur sapa dengan ibu itu sedangkan itulah yg selalu i buat if masuk dat ward...

berat makin menyesak bila petients lain yang seward dengan ibu tersebut bercerita bagaimana ibu tersebut melalui malam yang berat dan menghembuskan nafasnya yang terakhir dan akhirnya apneu sendirian....

beliau xde saudara mara, anak juga xpunya, seminggu lepas dibawa ke polikinik bedah oleh pengurus pusat penjagaan org tua....

dan selama dia di ward, dia mmg sendirian, mengurus dirinya sendiri. sedih bukan?

sewaktu di poliklinik, i was the one yang check up beliau. beliau bercerita, hidup masa mudanya susah, kerja berat dan mungkin itu lah penyebabkan berlakunya prolapsus tersebut...

masih terngiang2 ditelinga, si ibu mngadu kat i, "saya mau ini dibuang sahaja, sudah ga tahan neng(cik)"

namun walau tampak berat badan kurus nya mmbawa usus yang hampir sepanjang 20 cm yang keluar dari anus nya dan semakin berat apabila rahim sebesar dua kepalan tangan bergantungan di celah kelangkangan beliau, namun muka tuanya tak pernah lepas dari senyuman. manis! ibu tersebut sungguh manis! semanis kek-cawan....

bahkan mungkin lebih!

ps: al-fatihah..... semoga ibu tetap manis dan bertambah-tambah manis di sana....

bapak, saya khuatir urine retention yg bapak alami sekarang adalah disebabkan oleh benign prostatic hyperplasia(BPH) sebagai differential diagnosis drp urolithiasis sbb pada usia bapak, mmg sering terjadi BPH. jadi saya harus melakukan pemeriksaan rectal touche jd saya harap bapak dpt berikan kerjasama....

i have to say this:

bapak....**eerrr-ing for awhile sbb mencari ayat yg sesuai**

di dalam perut bawah bapak itu ada organ yang namanya prostat, sebesar ini(lalu cik dokter wat gaya tangan kira2 sebesar normal prostate), ditengah2 organ ini, dilalui oleh salur kencing pak. pada usia bapak, organ ini bisa membesar dan mengeras lalu menjepit salur kencing bapak. makanya bisa mnyebabkan bapak ga bisa kencing selain dari kemungkinan adanya batu buli(bladder). jadi saya harus melakukan pemeriksaan pada bapak untuk membuktikan bahawa masalah kencing bapak ini disebabkan oleh prostat yang membesar dan mengeras atau bukan.

karena organ itu di dalam**hela nafas panjang** jadi saya harus mencoba untuk merabanya dari dalam pak. caranya saya harus memasukkan tangan saya ke, maaf ya, lubang d*b*r bapak. tp bapak ga usah kawatir, ga sakit koq bapak. mungkin agak tidak nyaman tapi sebentar sahaja koq pak.... saya minta kerjasama bapak ya.......**hela nafas panjang lagi**

yes! bukan senang nak explain mende kat patients, esp mende yg agak sensitive mcm ni, lagi laaa kene hati2 mengexplain nye (=,=')

everything went well... very well indeed...

the foetus was more or less 8cm long..

he was still moving! i saw his legs kicking!

and i saw his chest pumping...

then he stopped n never move again...

then akhirnye when the surgery ended

we took the mother to the recovery room

she asked for her foetus

the dad da amik da the foetus

he even took a picture of it before we put it into a container and give it to the family..

then we asked her whether she wants to see the picture of it

then die geleng at the same time air mata die menitis

aduh.. sedih nyee..

this is that lil' strong fella. we took a picture of it because it's very rare utk dpt keluarkan foetus secara whole mcm ni

it's an amazing experience for me...

In addition, Husband 1.0 uninstalled many other valuable programs, such as

Romance 9.5 and Personal Attention 6.5, and then installed undesirable programs such as

NBA 5.0,

NFL 3.0 and

Golf Clubs 4.1.

Conversation 8.0 no longer runs, and HouseCleaning 2.6 simply crashes the system.

Please note that I have tried running Nagging 5.3 to fix these problems, but to no avail.

rase nak patah tangan uli pastry die(exaggerate!)

wat smpai 6 dough so leh agak la bnyak mana kan

tetibe ain ajak buat this cheesy creamy yummy(even tho not that healthy, haha) strawberry tart

siyes sedap

the pastry pun simple tp just nice rase die

and the filling even tho just pakai campak je sbb nak abiskan extra ingredients lebihan buat brownies the other day and surprisingly it turned out very3 well!

kasik tabik spring skeeeett!

Cheesy Creamy Yummy Strawberry Tart

pastry:

2 cups all purpose flour

1 pinch salt

10 tablespoons butter, diced

1/4 cup confectioners sugar

1 egg

2 tablespoons cold water

filling:

1/2 cup sour cream

1/2 cup whipping cream

150g cream cheese

4 tablespoons sugar

lemon juice

fresh strawberries(diced and wedged)

1 tablespoon strawberry jam

5 tablespoons hot water

how to(pastry):

dalam mangkuk besar, letak flour, salt, then butter. mix it all using a fork(or watever tp jgn guna tangan nnt butter tu lembik) then add up the confectioners sugar. bila the mixture dah crumble, kocokkan telur dgn cold water tersebut then mix it up with the rest of the ingredients. uli smpai die jadi one whole dough. warp dgn plastic wrapper, then simpan dlm fridge for 30 mins

how to(filling)

campak everything dalam one mangkuk and then whisk smpai smooth then masukkan strawberry yg diced tu. the strawberry jam, cairkan with 5 tablespoons hot water then let it cool down...

how to(whole tart):

bila pastry tu da 30 mins dalam fridge tu, kuar kan, buat small balls, then penyek2kan dalam tart tray. bakar for 20 mins(or less) dgn suhu 200 degree. bila da kelihatan brown skit, keluarkan and let it cool down first. then, spoon in the filling, hias dgn the wedged strawberry and akhir skali, spoon a little bit of the strawberry jam onto the wedged strawberry.. time ni the filling will be agak cair, but dont worry sbb die mmg mcm tu. put the whole batch of the tarts in the fridge and let them rest for the whole 4hours. the filling akn jd keras nnt... boleh la di serve.. yummy!

thnx ain for the recipe. but i change it a little here and there tp rasenye tak mnyimpang jauh kan ain kankankan? hehehhe~

marah nye ngan nurse td tu!

230pm, 30mins sblm abis shift today, our patient kejang2, and has gone sopor

she's a type 2 diabetic patient and telah mngalami metabolic acidotic state!

1st thing yg kite kne pikirkan adelah stopkan kejang tersebut

and system buduh kat hosp in indonesia ni mnyebabkan we have to wait so long for the daughter of the patient to go down to the pharmacy to buy the diazepam! ngok betul diazepam takde simpan kat ward!!

da dpt diazepam, terhegeh2 plak si nurse tu sbb shift die da nak abis kot

last2, my friend and i terpaksa ingatkan the other nurse yg baru masuk jaga to give our patient the diazepam and what did she said???????,,,, "owh? belum di kasi ke?"... how vavi is that???

and mase tu, the patient's situation da sngat teruk da! marahnye!

kejang for 1 hour or more?! pk snirik la siksa nye mcm mana!!! kite cramp kaki skit pun da merengek2, bayangkan kejang smpai mata terbeliak ke atas n smpai terberak2 ape perasaan!

vavi la nurse tu! muke ketat namampos! berlagak bos! and she put the patient's life in jeopardy!!! bayangkan la kalo ur mom yg lie there on the bed in that ward??? and org wat die mcm tu. oh gosh marahnye! mentang2 la it's a government's hospital, and of course mostly yg dtg situ sume org susah and tak bayar, so u can treat them like sh*t la??? vavi la kan??!

'nuff vavi-ness for today!

.::. makcik tu baik sngat. kesian tgok die kne lalui hal mcm tu... smalam siap berlawak2 ngan die lagi. and td pagi pun die still leh ckp ngan i and even smile, eventho ms tu die da demam tinggi and menggigil.. sigh..

tempat: IGD@Instalasi Gawat Darurat@Emergency Room

waktu/tarikh: 1-2am/27 april 2010

kejadian: a patient, Mr D, umur 25 tahun, dtg sbb terkepit kaki ms berkerja kat pelabuhan.. jari tengah kaki kiri da totally putus hanya lekat di kulit sahaja.. hantar x-ray, sah2 xdpt diselamatkan. kne buang. tgh mencuba menutup luka2 Mr D, terdengar suara Mrs M melolong2 menangis... owh, Adik R yg meningitis tu da meninggal.. last i sdiri check temperature Adik R malam td mmg tinggi.. 41.9 degree... then siap adik R seizure depan i lagi... uh.. tangisan Mrs M sngat menyedihkan! sambung menjahit kaki Mr D... tb2 IGD heboh! ade patient dr ward di lantai 8 baru lompat dan membunuh diri! mungkin mayat akan dibawa ke IGD!! wow! menarik! tunggu punya tunggu... xjadi! mayat trus dibawa ke kamar jenazah..

tempat: masih di IGD

waktu: 4++am

kejadian: IGD da quite lengang... melepak2 kat kaunter. kaki i da mmbengkak. mata da layu badan da sakit2 and kepenatan. masih cuba bertahan. datang 2org perempuan dan sorng lelaki ke kaunter dan lantas bertanya....

perempuan A: doc, Adik R yang tadi di sini, di mana ya?

nurse: owh sudah di kamar jenazah...

(muka perempuan A terkejut gila dan trus bertukar pucat)

perempuan A: loh??? kenapa???

(nurse mula serba salah)

nurse: eeerrr... udah meninggal...

(dan perempuan A pun jatuh terjelepuk di lantai di tengah2 IGD dan melolong2 menangis)

perempuan A: anakkuuuuu!!! anakkuuuuu!! ga mungkiiiiinnnn!!!

mungkin die balik utk rest kejap kot sbb muka die mcm baru bgn tdo... kesian die... takde mase anak die meninggal. sedih!

.::. belajarlah menyayangi ape je yg anda miliki skrng. org yg tersayang, anggota tubuh mahupun nyawa sbb kita tak tahu bile mendetu akn hilang dr kita...
